"I have prepared this information so that I may spare you from the numerous decisions that must be made at my death.

Within these pages I have recorded my final wishes, and ask you to take this to the funeral director I have named, so that these requests may be fulfilled.

For your benefit, I have also listed the vital information you will need, along with a list of documents you may wish to retain for your own purposes.
